Application Forms

You can apply for an UnLtd Sport Relief Award online by following one of the links below and completing the simple step by step form. Alternatively you can download an application form, fill it in by hand and send it to the address below.

If you are applying as a group you will need to decide who your lead member is, they will take on greater responsibility for the project. The lead member should complete the lead individual application form and all other members will need to fill out a group member form.

If you are applying as an individual simply fill out the lead individual application form. Do not complete the group member form as this will be returned, and you will be asked to complete the lead individual application form.
